South County Community Health Center, Inc.,dba Ravenswood Family Health Center, is a nonprofit federally qualified community health center based in East Palo Alto. We provide healthcare for the underserved, uninsured and most vulnerable low-income residents of communities in southeastern San Mateo County.
Our purpose as a community health center is to remove those barriers -whether financial, geographic, linguistic or cultural –that prevent people from seeking and receiving health care. Click here to read History of Community Health Centers >>
Integrated and Comprehensive Model of Care
Since our opening in December 2001 we have established an excellent model of healthcare that integrates primary medical care, prevention and health education, behavioral health counseling and social services and full scope dental services.
We operate two medical clinics, one in East Palo Alto and the other in the Belle Haven neighborhood of east Menlo Park. Our Ravenswood Mobile Health Clinic, a gift of Lucile Packard Children’s Hospital, makes services available at schools and once a week for the homeless at sites where they congregate.
In April 2010, we opened Ravenswood Family Dentistry, a state-of-the-art full-service dental clinic in East Palo Alto. Next door to it is our new Center for Health Promotion which features a branch of the Stanford Health Library.
