Ravenswood is a Federally Qualified Health Center (FQHC) located in East Palo Alto, California, that provides over 50,000 medical visits/year to the residents of South San Mateo County. Registered Nurse - Under general supervision the primary function of the RN is to utilize the nursing process to provide direct and indirect support to the clinician in the delivery of patient care or to provide direct patient care. S/he accurately administers medications and immunizations as well as providing patient education and overseeing the provision of education by MAs on the clinical team. S/he performs independent triage and communicates with the clinician regarding actions taken. RNs are expected to demonstrate critical thinking skills, leadership and appropriate role modeling. Behavioral Health Staff
The Licensed Clinical Social Worker (LCSW) will be assigned to provide on-site brief assessment and intervention, staff consultation, member de-escalation and conflict resolution, bridging to local community social services, and staff trainings to the Boys and Girls Club of the Peninsula staff, members, and families. This position works primarily weekdays in the afternoons/evenings, is a member of RFHC’s Integrated Behavioral Health Services (IBHS) team and co-located out of the Boys and Girls Club of the Peninsula. The LCSW will report to the IBHS Administrative and Clinical Directors As needed the LCSW will provide services at RFHC’s primary care sites and maintain an appropriate caseload. Pediatric Dental Health Navigation - Ravenswood Family Health Center is seeking a Pediatric Dental Health Navigation Intern who would work to enhance integration of services between our pediatric primary care program and dental practice. The PDHN Intern will serve as a link for families by providing navigation and warm hand-offs between our pediatric primary medical program and dental clinic.
