Get Fit EPA
When RFHC asked other community groups to help jump start
an all-out community campaign to address the rise in the number
of overweight people, lots of community groups jumped in with
great ideas and GET FIT EPA was born.
East Palo Alto Asthma Taskforce
- Improve quality of life for patients living with asthma.
- Advocate for policy and environmental change
- RFHC Medical Assistants teach "Open Airways" to help
school children learn how to prevent asthma attacks
Health Disparities Collaborative
RFHC is a member of the Pacific
West Coast Health Disparities Collaborative (HDC) sponsored
by the California Primary Care Association. Our Health Disparities
Team chose to focus on diabetes, which is very common among
our patients. Their goal is to implement the Health Disparities
Collaborative's chronic care model that takes a team approach
and documents key diabetic measures for every diabetic in a
patient register which allows us to evaluate a patient's progress
over time.
