Partners in Pursuit of Community Health
At Ravenswood Family Health Center, partnerships are key to providing the highest quality care to an ever-expanding patient population. By involving public agencies, private institutions, foundations, and individuals, we can multiply the resources and reach of our programs. We are fortunate to have medical partners, government partners, and community partners who collaborate with us on projects to improve the health of our patients.
Government Partners
Health Resources and Services Administration: Bureau of Primary Health Care
A grant of $1,011,128 covers about half of the cost of primary care for the uninsured.
The California Department of Health Services funds immunization services for RFHC and community residents as well as Expanded Access to Primary Care.
Medical Partners
- Lucile Packard Children's Hospital contracts pediatric providers to RFHC.
Our patients are referred to LPCH for Obstetrics and pediatric specialty or emergency care. LPCH also conducts its 24-week Pediatric Weight Control in both English and Spanish
- San Mateo Medical Center is the referral for adult emergency care and speciality services including mammography, imaging, X-ray
- Palo Alto Medical Foundation provides pro bono quality assurance and IT consulting
- RFHC is a Family and Community Medicine rotation site for Stanford Medical School students
Community Partners
El Concilio of San Mateo County
As an RFHC partner El Concilio contracts a Community Health Worker to work on our chronic disease management team.
Get Fit EPA
To address the rising number of overweight children and adults in our community, RFHC initiated and coordinates a collaborative, Get Fit EPA. Our mission is to improve the health of community residents by increasing awareness, resources, and promoting healthy eating habits and physical activity. Our partners include:
East Palo Alto Asthma Taskforce
RFHC is a member of the East Palo Alto Asthma Task Force that seeks to address the causes of high asthma utilization of emergency room services by children in the South County. Statistics show that South County children utilize hospital emergency rooms for asthma three times more frequently than children elsewhere in the County. Several RFHC staff members are trained to facilitate Open Airways classes that teach children that have asthma how to prevent or cope with asthma attacks.
