Pediatric Weight Control
In our community health partnership with Lucile Packard Children's Hospital, RFHC is a pilot community site for the Pediatric Weight Control Program, a six-month family-based program designed to promote behavior changes that lead to more exercise, better diet, and improved family patterns. Two groups (English and Spanish) make up of 10 to 12 kids (ages 8 -12) with their parent/caregivers meet weekly for 24 weeks. Trained parent group leaders with a LPCH coordinator provide a series using creative activities, including exercise, to inspire and reinforce changes in eating and activity patterns. The program seeks to improve attitude, which is more significant than weight loss.
Healthcare for the Homeless
Our services for the homeless include street outreach, screening for diabetes, hypertension and primary care and dental services. Margaret Allen, Physician Assistant (P.A) and Ty Deldridge, Community Outreach Coordinator, seek out the homeless in parks or crack houses. In an average month, RFHC clinicians treat 85 homeless patients in 120 plus encounters.
A portion of the cost of care for the homeless comes from County's Health Care for the Homeless program.
Children's Health Initiative
San Mateo County Children's Health Initiative funds two Community Health Advocates at RFHC who enroll families and children in Healthy Families, Healthy Kids, and Medi-Cal. In 2005 RFHC enrolled 1001 children in health insurance programs.
Health Disparities Collaborative
Studies show that ethnic minorities suffer from poorer health and less favorable health outcomes than the general population. The difference between health indices for low-income and minority communities and more affluent communities is called "health disparities." Lack of health insurance, other barriers to health access, and environmental stressors all contribute to health inequities. The U.S. Department of Health and Human Services has made eliminating health disparities a national goal for the Healthy People 2010 Initiative. Here at Ravenswood, we are working to eliminate health disparities to the extent possible.
